Students with disabilities are asked to send us a copy of the Disability Services Medical Form.

 

This form must be signed by a medical practitioner.

 

The form can be mailed or faxed to the Teacher Consultant prior to your first meeting, or you can bring it with you on the day.

 

We do however, recommend that you try to submit the form before the first meeting as this will give the Teacher-Consultant time to make a number of enquiries and arrangements  prior to your first visit.

 

The Hunter Institute has a First Aid Officer on every campus.
